Case Manager-Don’t Fall Down In the Hood

JOB DESCRIPTION

 

Position Title: Case Manager

Reports To: Program Coordinator

Location: Don’t Fall Down In the Hood Program (@ Temple University)

Status: Full Time, Salaried Employee (+ Benefits)

Education: Bachelor’s Degree in Social Work or a related Human Service field, or Five years experience in nonprofit and/or community based human service agency.

 

Qualifications: Strong interpersonal and communication skills. Detail oriented. Good organizational skills with the ability to manage multiple and varied tasks. Proficient with computer programs: MS Office and Internet. Day and evening hours – some weekends. Travel required in and around the City of Philadelphia (Must own car with valid driver’s license).

 

Basic Functions: Function as a coordinating staff between the Institute and the Philadelphia Department of Human Services, Philadelphia Family Court, Youth Study Center, Youth Aide Panels, and the Office of the Philadelphia District Attorney. Conduct screening process of suitable program candidates and conduct in-home family assessments. Design and implement individualized treatment plans. Attend to necessary and appropriate client documentation.

 

Specific Responsibilities:

 

  1. Conduct initial/ongoing in-home family assessments.
  2. Screen/process all suitable candidates into the project.
  3. Develop an Individual Service Plan (ISP) for each client.
  4. Facilitate the development of an action plan in partnership with the family.
  5. Initiate timely and appropriate interventions as needed on behalf of the youth/families.
  6. Coordinate treatment plans and follow through with Family Courts, Youth Study Center, Youth Aide Panels, and youth’s schools.
  7. Collect necessary data for statistical purposes.
  8. Participate in ongoing program evaluation protocol.
  9. Prepare appropriate reports, as requested.
  10. Attend all internal agency wide and external meetings as relevant to the program
